This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Clinical Nurse Specialists. A clinical nurse specialist must possess a Master's degree from an academic program accredited by the NLNAC or CCNE [and maintain full and current certification as a clinical nurse specialist from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected.] Required: MSN Degree CNS Certification Preferred Criteria: Current BLS provider card Clinical experience in Geriatric nursing, which may include home care, long-term care and/or home hospice Minimum of two (2) years-experience in Dementia Care Experience with program development Gerontology (RN-BC) or Rehabilitation (CRRN) certification Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Physical Requirements: This position requires heavy lifting, 45 pounds heavy carrying, 45 pounds straight pulling (2 hours); reaching above shoulder; use of fingers; both hands required; walking & standing 8 hours - intermittently; repeated bending; both legs required; ability for rapid mental & muscular coordination simultaneously; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; depth perception; ability to distinguish shades of colors; hearing (aide permitted). Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The Community Living Center (CLC) Clinical Nurse Specialist (CNS) who provides a number of services to CLC residents of all levels of service, ethnicity, and gender assuring that the highest degree of resident's quality of life and care is maintained within a home environment. The incumbent is clinically and administratively responsible to the Chief Nurse Extended Care Service. The major duties and responsibilities include, but are not limited to: Assists residents and CLC team members with assessment/management of resident care issues related to dementia. Serves as a dementia clinical resources expert and functions as an educator to residents, caregivers, care teams, and community. Demonstrates expert practice that goes beyond the unit to encompass a program, an entire service, or across service lines and to improve clinical outcomes for veterans with dementia. Identify learning needs of the staff and provides formal and informal education and training with periodic reevaluation of their learning needs. Develops, implements and evaluates clinical policies, procedures. Acts as a resident/family advocate for the protection of resident/family rights. Serves as a leader to identify and contribute to the resolution of ethical/legal issues related to nursing practice in collaboration with appropriate resource people. Identifies and analyzes resources available for care, establishes baseline, and identifies risk areas. Addresses high risk areas and maintains safety standards for managing the unit/program safely to ensure a safe environment and meet all occupational safety & health requirements. Collaborates with nursing leaders to develop, implement and evaluate resident-centered and/or staff development programs. Develops and implements new/innovative programs for patient care in a collaborative manner Collaborates with multidisciplinary teams in planning, directing, and implementing a resident-centered environment. Systematically evaluates and facilitates change in clinical practice/standards of care. Responsible for assessing, planning, implementing, and evaluating the educational activities. Acts as a consultant in clinical practice: Provides for or serves as a resource to other staff in meeting individualized resident/SO learning and discharge needs, recommending appropriate referrals, adaptive equipment and approved educational materials. Meets established policies, procedures and standards of care in accordance with the Joint Commission, other regulatory agencies, and VA policy. Communicates and interprets organizational policies and procedures to the health care team. Functions as one of the key people in the development and implementation of policies and procedures related to the standards. Promotes evidence based clinical practice and assist staff and leadership with Quality Management and Performance Improvement programs, including research projects and development of improved models of care. Shares clinical/professional expertise with others within the facility or beyond (i.e. consultation, presentations, publication, mentoring advanced practice students, participation in professional organizations, etc. Collaborates with Informatics and Clinical Application teams to meet and update staff learning needs with electronic documentation. Utilizes methods, measurements, and tools to evaluate achievement of learning goals and to collect and analyze data. Enhances and facilitates clinical excellence and professional standards and performance through the participation in nursing grand rounds, continuing education programs, in-services, journal clubs, student experiences, new employee, and experienced staff orientation. Acts as a resource and liaison by serving on facility-wide committees and task forces. Works collaboratively with and is an active member of the Extended Care Nursing Leadership Team. Change agent, fosters and demonstrates innovative and creative approaches to problem solving and process improvement. Works collaboratively with CLC management to assess, develop, implement, and evaluate educational programs for staff, maintain and enhance clinical competence, and manage ongoing education and competency programs.
