The Registered Nurse, Patient Aligned Care Team (PACT), sees Veterans for care management and is responsible for ensuring their health are need in collaboration with the Core Team (Veteran, Primary Care Provider, LPN, and Clerical Associate) are met. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
