You will assist in the coordination of Care in the Community (CITC)between the VA and outside facilities and physician offices. Contact facilities/offices regarding patient appointments and obtaining medical documentation of care provided. Provide documentation to HIMS staff for scanning into the patient's medical record. Assist in reviewing and approving consults based on established criteria and service agreements. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
