This is a full performance level social worker for the Advance Care Planning via Group Visits (ACP-GV) program. The social worker is responsible for providing advance care planning services in group or individual. Services are provided face to face, VVC or phone. Incumbent will also complete psychosocial assessment and provide services. Title for announcement: Social Worker Advance Care Planning. HPTs who are on track to graduate/complete the requirements should respond as if they have met the requirements. Provide details in the narrative sections of the assessment.
