The VA Community Living Center (CLC) is located inside CHI Midlands Hospital, 11111 South 84th street, 5th and 6th floors. The CLC provides services to eligible Veterans needing skilled, rehab and dementia care, ranging from 30 - 90+ days of care. The CLC is focusing on a team approach to produce specialized nursing staff, to be knowledgeable in a variety of areas from standard nursing care, wound care, rehab, pain management, restorative care, hospice care, dementia and extended care services.
