The RN-Intensive Community Mental Health Recovery (ICMHR) Case Manager functions as a primary Case Manager, consultant, and educator for outpatient Veterans assigned to ICMHR clinical services. The duties and responsibilities for the position includes but are not limited to the following: Provides Clinical services for Veterans with severe mental illness. Performs Site visits in the Veterans' homes, care homes, or assisted living facility. Ensures that Veterans are stably housed and successfully integrated within their communities. Coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery and linking Veterans to the VAMC. Recognizes the complexity of care required in the psychiatric patient and can assist with developing a partnership to employ patient goals that may renew hope, involvement in meaningful activities, becoming empowered, overcoming social barriers to inclusion, and managing symptoms. Assesses, plans, implements, and evaluates care based on age and cultural-specific components. Provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. Provides cross coverage to other areas as assigned by management. Influences care outcomes by collaborating with members of the interdisciplinary team. **Government Driver's license required within 30-days of EOD **Position requires entering Veteran's homes and accompanying Veterans to outings Other duties as assigned.
