The incumbent practices as a RN within the Community Living Center (CLC), located at the Vancouver campus in the Rehabilitation and Long Term Care (RLTC) Division. The RN provides short term transitional care for residents who require skilled nursing care, close medical monitoring and sub-acute rehabilitation. Resident populations include those having chronic or sub-acute medical conditions, including residents that are chronically ventilated. The duties and responsibilities of the position include but are not limited to the following: Providing care and counseling of patients. Promoting and maintaining the health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and Evaluating human responses to actual or potential health problems through case finding, health teaching, health counseling, provision of care supportive to or restorative of life and well-being. Executing medical regimens including administering medications and treatments. Executing position responsibilities that demonstrates leadership, experience, and creative approaches to management of complex patient care. Leading and organizing delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning. Using advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating outcomes. Functioning as an expert in clinical practice and/or areas related to the assigned roles and responsibilities. Systematically evaluating current practice, and formulating outcomes for groups of patients and/or organizational processes within an area of expertise. Guiding, developing, and supporting staff from a leadership perspective. Using professional standards of care, scientific evidence, and practice to evaluate programs and/or service activities. Demonstrating expertise in leadership at the systems level that is sustainable, experience, and creative approaches to management of complex situations. Leading and organizing delivery of services to assure continuity and peer accountability for practice. Practices good customer service in all work activities, treating veterans, their families/representatives, visitors, and other employees in a courteous, tactful, and respectful manner. Embraces a continuous culture of safety for veterans, their families, visitors, and other employees. Protecting printed and electronic files containing sensitive data in accordance with the provisions of the Privacy Act of 1974 and other applicable laws, federal regulations, VA statutes and policy, and VHA policy. Protecting the data from unauthorized release or from loss, alteration, or unauthorized deletion. Reporting all known information security incidents or violations to the supervisor and/or the Information Security Officer. Reports all known privacy incidents or violations to the Privacy Officer immediately. Providing short term transitional care for residents who require skilled nursing care, close medical monitoring and sub-acute rehabilitation. Accountable for patient driven care of a defined group of residents. Based on education and experience, the RN may be assigned as a charge nurse, preceptor; or in the interdisciplinary leadership roles of Veteran Care Manager, Minimum Data Set Coordinator, or Admission Coordinator.
