This position, located in Kerrville, Texas, is the Minimum Data Set (MDS)/Resident Assessment Coordinator (RAC) Community Living Center (CLC) for the Audie Murphy Division. Minimum Data Set (MDS)/ Resident Assessment Instrument (RAI) Program Resident Assessment Coordinator (RAC) for the Audie Murphy Division will: Demonstrate a high level of clinical, managerial and leadership skills, have knowledge and adhere to Veterans Health administration policies, procedures and evidenced based practice in Long Term Care standards and the Center of Medicare and Medicaid Services (CMS) and its application to the RAI-MDS process. The MDS/RAC must be self-directed and effectively implement health promotion, prevention, and management of skilled care, rehab and chronic illness, and care of the dying. The MDS/RAC must possess strong administrative skills; to include the ability to manage and coordinate the facility RAI-MDS Program. The MDS/RAC is responsible for the oversight in the coordination and facilitation of the RAI-MDS process/program, demonstrate a high level of clinical, analytical and leadership skills and is competent in conducting and reporting quality measures and performance improvement activities. The MSD/RAC is responsible for the RAI-MDS program process. The Facility Director, GEC Leadership, CLC Medical Director, and ACNS is accountable for changes in the program that effectively support the program and is responsive to the Quality Measures and other Performance Measures.
