The VA Northeast Ohio Healthcare System is recruiting a full-time Sleep Physician to work in a large tertiary care academic medical facility. Qualified candidates will be board certified/board eligible in Sleep Medicine. This position will include providing direct patient care in a thriving ambulatory care setting within an academic medical center, as well as supervising nurse practitioners, sleep technicians, respiratory therapists, and sleep medicine fellows. Candidates will demonstrate expertise in PSG reading, PAP therapy as well as home sleep studies. An interest in clinical sleep research with an established track record of research and publications is preferred. All candidates will be eligible for an academic faculty appointment through Case Western Reserve University School of Medicine. The incumbent will serve as a Physician in a Patient Aligned Care Team (PACT) setting. He/she will be responsible for providing medical care in an outpatient setting following PACT principles. May provide medical care using Telehealth for remote patients, or travel to other CBOCs to cover provider shortage. Duties include, but are not limited to: Utilizes all available tools, such as registries, to enable effective and efficient identification and intervention of individual patients and cohorts. Assures that clinical care provided meets all timelines and required documentation for the Chief of Staff (COS) office. Participates in multi-disciplinary team meetings to achieve a patient centered, holistic approach for clinical care and needs of the Veteran, focusing on continuous improvement of critical team processes. Ensures the patient's care plan contains medical recommendations for clinically indicated care. Ongoing, continuous care of one or more assigned panel(s) of Veterans providing health education and health coaching on wellness, disease prevention, chronic care management and self-management skills to patients and personal support persons. Collaborates with informatics technology staff to develop and implement systematized, electronically supported, standardized, tools to support PACT care delivery processes (e.g., pre-visit reminder calls, post hospitalization follow-up calls, recall scheduling procedures, new patient orientation, disease registries and primary care protocols for chronic disease management. Work Schedule: Full Time Tour of Duty: Monday - Friday; 8:00 am to 4:30 pm with extended hours every other Thursday; 10:00-18:30. NOTE: Candidates may be required to work weekends, irregular work hours and extended hours. The candidate must be willing to be assigned to other related services and/or locations, if necessary. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.
