The VA Hudson Valley Health Care System is currently recruiting for one (1) Full-Time Registered Nurse - Resident Assessment (RAC) Coordinator. The RAI-MDS Resident Assessment Coordinator (RAC) in Nursing-Rehab & Extended Care is a self-directed, quality specialist nurse with a broad-based clinical knowledge appointed to effectively implement health promotion, prevent sub-acute illness, manage chronic illness, and provide hospice care. The RAC coordinates/facilitates interdisciplinary treatment team meetings, assisting in the development of the resident's care plan and ensuring individualized long-term and short-term goals are identified for each resident. She/he is responsible for the coordination, tracking and completion of all MDS assessments for all residents on the CLC. The RAC is a member of the Extended Care management team. The RAC works closely with the Chief Nurse, Geriatric leadership and CLC nurse managers, initiating and facilitating evidence-based practice and research, and providing leadership in the ongoing systematic evaluation and improvement of the functions of the MDS process. He/she facilitates an interdisciplinary approach to high quality, cost effective and customer focused health care. Position may include other duties as assigned and travel to both campuses. Responsibilities include but are not limited to: Coordinate/facilitate the facility RAI-MDS process according to CMS regulations and guidelines. Ensuring the completion of care plans based on MDS assessments. Guide and support the interdisciplinary team in the performance of inter-rater reliability Providing expertise and knowledge in promoting the application of the RAI-MDS assessment and care planning process, considering the age specific needs of the residents. Timely review and audits of MDS assessments and coding, guiding the interdisciplinary team in the appropriate plan of action related to quality findings. Develops and implements an educational plan for all involved in the MDS process to improve role performance. Ensures RAI-MDS training for all new staff in extended care with initial and ongoing competency as needed. Timely transmission of the MDS information. Timely communication to leadership and the interdisciplinary team any changes to directives/handbooks and CMS guidelines that pertain to the RAI-MDS process. Identifies and implements best practices as related to care, quality and cost associated with the RAI-MDS process. Participate in VHA RAI-MDS conference calls, and other VISN or national calls related to MDS. Facilitates investigation of data collected and develops benchmarking or other qualitative and quantitative techniques to improve clinical care criteria, standards and practice related to the CLCs. This position reports directly to the Chief, Nursing, Rehab & Extended Care but practices independently, exercising initiative and independent judgement in day-to-day activities.
