The incumbent, an independent Social Worker, functions as one of the Social Workers for the Medical Center's HUD/VASH (Housing and Urban Development/VA Supported Housing) Program. The HUD/VASH program is a collaborative effort, supported through HUD Section 8 rental assistance vouchers and VA's provision of intensive case management services. The primary goal of HUD/VASH is to move Veterans and their families out of chronic homelessness. The primary component of the program is VA case management services, designed to improve Veteran's health and mental health, enhancing their ability to remain stable, housed and community-integrated. The incumbent works closely with the VA medical center's Housing Program, residential and community care programs, and domiciliary programs to ensure that coordination between these entities is facilitated and Veterans direct care, referral, and follow up is contiguous and based on a continuum of services. Duties of this position include, but are not limited to: Establishing collaborative relationships with community organizations to assist in operating the program, specifically the local Public Housing Authorities with collaboration with the HUD/VASH program. Reviewing agreements between Section 8 landlords, Public Housing Authorities and the VA to establish appropriate referral sites for placement of Veterans in housing. Conducting and participating in outreach activities including education of community groups about homelessness and Veteran's health issues addressed with the HUD/VASH Program. The incumbent assists Veterans in maintaining appointments at VA medical centers and other community agencies for medical, psychiatric, substance abuse, vocational and other services consistent with the Veteran's goals associated with independent living. The incumbent refers Veterans to appropriate VA and other government/community services-including local shelters, food banks, employment services, Veteran's benefits, and social security applications. The incumbent assists Veterans who encounter problems obtaining services from the VA and other service delivery agencies through advocacy, linking, and/or by assisting with their problem solving/social interaction skills. Providing initial engagement and rapport building to Veterans entering HUD/VASH. Conducting screening, assessment, and eligibility determination for HUD/VASH. Providing either individual or group therapy for Veterans participating in the HUD/VASH Program in combination with the Community Outreach and Housing Integration Specialist. The incumbent establishes initial treatment plans based on clinical assessment for each Veteran. The incumbent makes routine visits to each Veteran in residence to assess their environment and provide on-going clinical services. Assists with triage of case mix, accepts assignment of more complex cases, and provides more intensive case management services to Veterans whose constellations of social problems may significantly impact ability to manage independent living without a strong case management presence.
