This positions in located within the Long Term Care section of Nursing Services. The following section make up Long Term Care; 5A/5B, 5C, CLC-A and CLC-B. Demonstrates ability to function effectively in charge nurse, team leader, or other leadership roles requiring application of problem solving skills at the unit or team level, and uses team approaches to identify, analyze and resolve problems. Serves as a role model in assessing and evaluating patient status and identifying priority needs for care. Develops the initial plan of care and updates the plan, serving as a resource person to less experienced staff, and acts as a leader in developing standards of care. Includes patient/family/significant other/health care providers in establishing realistic, measurable goals related to patient education and treatment plans and in planning care on a continuum; assists staff in the process of mutual goal setting. Demonstrates skill in implementation of nursing and physician orders for patients with complex nursing care needs. Documents and evaluates administrative and clinical information in a thorough and accurate manner. Utilizes problem solving methodologies to resolve problems that impact on patient care or staff performance. Considers the cognitive, physical, emotional and chronological development of adult and geriatric patients while providing patient care.
