The San Francisco VA Medical Center, Social Work Service is seeking one full-time Rehabilitation Counselor. The Vocational Rehabilitation Counselor CWT, Community Based Employment Specialist (CBES), provides vocational counseling and case management for Veterans with a variety of conditions. This announcement will be open until filled. First cutoff will be on 12/04/2020. After that, applications will be reviewed every two weeks until position is filled. The VRC CBES conducts initial vocational rehabilitation evaluations, entitlements determinations, problem solving counseling, case documentation, and therapeutic employment services to include skills training and development, employment support services, and job development and placement services, to Veterans diagnosed with mental health disorders, Substance Use Disorders, and other co-occurring disorders. The VRC CBES works closely with members of an interdisciplinary team of VA and non-VA providers to assist Veterans in obtaining and maintaining suitable employment. The VRC CBES works with community-based employers and agencies to build on Veterans strengths and vocational interests. The VRC CBES utilizes Motivational Interviewing techniques and gathers information to assist Veterans in identifying occupational areas of interest and job search activities. VRC CBES assists Veterans with job seeking skills and strategies and understanding financial benefits entitlements programs and requirements. VRC CBES continues to work with Veterans, interdisciplinary teams and employers to provide ongoing support and feedback to Veterans and employers. VRC CBES assists Veterans with transferrable skills analyses to aid Veterans within pursuit of their self-determined employment, educational and independent living goals. In coordination with Veteran's interdisciplinary teams (IDT), develop and amend patient-centered treatment plans utilizing specific, measurable, agreed upon, realistic and time-based (SMART) goals.
