Reporting to the System Director of Value-Based Operations, the Post-Acute Care (PAC) Director will oversee the strategic direction for post-acute care including network development, quality performance, and accountability through collaboration with Baylor Scott & While Quality Alliance (BSWQA) departments and Baylor Scott & White Health (BSWH) acute care management.
The Director will ensure the PAC network is providing high-quality, low-cost care to BSWH patients and members. He/she will serve as the subject matter expert in post-acute payment regulations, help operationalize PAC needs for new payor contracts, and be the QA liaison for joint venture partnerships. The Director will lead the PAC team and provide briefings to the System Director and BSWQA leadership team as requested.
Essential Functions - Lead and develop the BSWQA PAC team - Oversee the development and performance of BSWQA Post-Acute network: - Long Term Acute Care - Inpatient and Outpatient Rehabilitation - Skilled Nursing Facilities - Home Health - Hospice - Ensure network adequacy for all levels of care in all regions - Hold partners accountable to QA expectations - Develop and own PAC reports and measures - Oversee PAC Subcommittee - Facilitate strategic discussions with post-acute joint venture partners - Analyze and disseminate PAC payment regulations - Oversee SNF Waiver strategy and operations - Facilitate BSWH PAC stakeholder alignment (inpatient care management, physicians, service lines, medical groups, etc.) - Develop COVID-related efforts as needed
Minimum Requirements - Bachelors Degree required - MBA or MHA strongly preferred - 5 years experience, preferably leading a team in a post-acute care environment
