LEAD CLINICAL ASSESSMENT SPECIALIST COMMUNITY LIVING SERVICES
Job Scope: The Lead Clinical Assessment Specialist provides a mixture of direct-service to clients and clinical consultation to the Front Door Assessment team. Actively contribute as a team member by providing strengths-based, client-centered, and trauma-informed assessments and services to adults with mental health conditions and co-occurring substance use disorders discharging from hospital and other settings. Provide consultation and clinical oversight to team members providing hospital assessments and making discharge recommendations for the “Front Door” Diversion program. Will complete comprehensive assessment, outreach, referrals, and treatment recommendations to assist adults avoid nursing facility admission and maintain in a lesser level of care and remain in community-based settings after experiencing a mental health crisis. Treatment services are delivered at flexible times that meet clients’ needs and in settings that offer the best potential for positive outcomes, including in hospital and nursing home centers, in-home, and in community-based settings. Coordinate with KYC team members, with network of hospitals, PAS agencies, and other local community mental health services.
Primary Responsibilities Clinical Duties:
Be available for same-day clinical consultation to team members providing hospital assessments.
Provide clinical consultation for post-discharge clients when solicited by team member, or as requested by supervisor.
Maintain a high level of personal and team documentation compliance including timely review and signing of team members’ integrated mental health assessments and treatment plans (IM-CANS).
Act as contact for landlords, hotel staff, and CST crisis line to problem solve response issues and call management for Front Door clients.
Review hospital referrals and work with outreach team to provide assessment and treatment recommendations to adults with mental health needs discharging from hospital settings.
Provide on-site intake at various community access points, by providing mental health assessment, treatment recommendations, and discharge planning activities in hospitals, at nursing homes, in the community, and other locations as assigned.
Provide outreach, case management, linkage and referral services to individuals discharging from hospital settings for up to 180 days to assist clients to continue to avoid institutional levels of care.
Develop housing alternatives, including relationships with local group homes, CILA, TLP, etc
Collaborate with partners to facilitate referrals and access to resources across broad geographical area.
Conduct phone check-ins and/or in person meetings to ensure client contact
Perform and participate in crisis intervention when necessary.
Meet regularly with CST, CSI, and transition team members to integrate care provision and share clinical responsibilities so that adequate coverage is available to clients at all times
Routinely consult with other Adult Team members including employment services, psychosocial rehabilitation, SUPR/MISA, drop-in staff, and psychiatry to address specific client related clinical issues and coordinate care strategies
Participate in Adult Recovery Team, Outpatient team, and Housing team meetings in order to generate referrals and provide feedback on individual community support services
Complete all clinical documentation and required reports in an accurate and timely manner
Interpret and communicate, at the service delivery level, Center policies and procedures
Meet or exceed productivity target
Participate in orientations, all staff trainings, and training and development seminars
Submit, in a timely and accurate fashion, schedules timesheets, mileage and other reports required by the Center’s policies and procedures
Assess client eligibility and complete applications with clients for pharmaceutical assistance programs, housing assistance programs, LINK/food stamps, Medicaid, and other available benefits.

Demonstrate the ability to consistently meet compliance requirements
Demonstrate understanding of chronic illnesses frequently experienced by those diagnosed with severe mental illness
Demonstrate knowledge of strong behavioral assessment and treatment planning
Demonstrate knowledge of specialized treatment needs of the populations served and the rehabilitative/recovery service approaches and interventions used to address these needs
Demonstrate knowledge of community resources to support client’s in independent living.
Demonstrate knowledge of specialized approaches to those experiencing chronic homelessness, substance use issues, and mental health difficulties.
Has ability to effectively engage clients in care
Ability to complete assessments to determine alternate care options from nursing facility admission
Skill in the use of motivational interviewing, harm reduction, and engaging clients in self-directed care
While performing the duties of this job, the employee regularly works in an office setting.
A majority of the employee’s time will be spent in the community or hospital settings, driving to and from locations, and providing case management in the home or community areas
In supporting clients to live independently, employee may be exposed to varying home conditions and drive clients in their vehicle
The employee is required to travel by automobile to and from Mt. Prospect, and Elk Grove office locations on a weekly basis, as well as frequent client visits at their homes, 8 area hospitals and nursing home facilities in the Chicagoland area, including suburban Cook, Dupage, and Lake County, and occasional meetings/trainings in and around the Chicagoland area.
While conducting external KYC business or traveling between sites, the employee may be exposed to fumes or airborne particles as well as changing weather and road conditions.
Full Time, Exempt, 37.5 hour work week
Must work a flexible schedule in response to the needs of the target population served, including rotating direct on-call responsibilities, which includes evening, weekend, and holiday hours.
Education and Experience
Masters Degree from an accredited College or University in Social Work, Psychology, Counseling or closely related discipline
LCSW or LCPC Licensure Minimum of 3 years of experience working with adults with serious mental illness
1 year experience working in non-traditional treatment environments i.e.in-home, residential, hospital, community settings
1 year experience working with individuals with chronic homelessness and/or substance use difficulties
IM-CANS certified or obtain within 30 days
Applicant must have a valid IL Driver’s License, proof of insurance, and daily access to a reliable well maintained vehicle
Basic computer and smartphone proficiency, knowledge of Word, Excel, and internet skills
