The Weld County Nurse-Family Partnership (NFP) program provides case management and education through home visitation for low income, first-time pregnant women and their families. Nurse home visitors conduct home visits from the early prenatal period to the child's second birthday. The Weld NFP program is a voluntary home visiting nursing intervention model that strives to improve critical outcomes for families in the community. NFP nurse home visitors establish and maintain therapeutic relationships while delivering individualized case management and educational services based on research-based NFP curriculum. Assessment of health, family strengths, goals, use of community services, and educational needs is an integral part of the NFP model. In addition, the nurses assess social support and promote the involvement of other client family members during pregnancy, birth, and early childhood. Research has shown that the NFP program has multigenerational positive health and social outcomes for families and the community. This Community Health Nurse will be assigned to the multidisciplinary community health services team and will work to provide individualized in-depth case management services to identify and address the medical and psychosocial needs of children, mothers, and families and to connect them with appropriate resources and services. This nurse will establish therapeutic relationships with each client and provide physical/educational assessments, health promotion, and health education, according to program guidelines. Additionally, this Community Health Nurse will participate in public health emergency response activities. Training will be provided.
MINIMUM QUALIFICATIONS Education: • Bachelor of Science degree in Nursing.
CERTIFICATES, LICENSES, REGISTRATIONS: • Current Colorado nursing license in good standing required.
• A valid State of Colorado driver's license and use of a personal, insured vehicle will be required to perform job duties. Auto insurance, maintained at all times, as required by state law.
RESIDENCY: Must reside in the State of Colorado on the date of hire.
BACKGROUND INVESTIGATION: Candidate must be able to pass a criminal background check. A pre-employment criminal background check will be conducted as part of the selection process. Felony convictions or convictions of crimes of moral turpitude or convictions of misdemeanors related to job duties may disqualify you from being considered for this position.
