Focus of the Quality Administrator is to provide oversight and quality improvement for Williams and Colbert class members. Quality Administrator is to remain current with trainings, monitor documentation, and provide oversight of all Williams and Colbert’s processes and issues. Ensure class members are being supported living independently and engaging fully in community life. Quality Administrator will be an active team member, monitoring aspects of the transition and services being received by Williams and Colbert class members.
Actively participates in DHS/DMH Williams and Colbert trainings, quality initiatives, Quality Improvement Committee meetings, and any other related DHS/DMH Williams and Colbert quality initiatives deemed necessary
Acts as the single point of contact with DHS/DMH regarding assigned Williams and Colbert class members and issues
Receives referrals of Williams and Colbert Class Members and assures assignment of class member cases
Provides oversight of all Williams and Colbert processes and issues
Completes routine quality reports and reports to DHS/DMH in prescribed formats and timelines to facilitate DHS/DMH reporting to the Court Monitor and the Court
Advises DHS/DMH if a Williams or Colbert class member changes their mind and elects not to transition into community placement
Monitors multiple aspects of the transition and services being received by Williams and Colbert class members, such completion of the Comprehensive Service Plans, development of the Treatment Plan and the actual services provided to the class member post transition, as well as the consumers stability, wellness in the community (housing management, medication management, eating, socialization, etc.) and ongoing mental health services provided.
Participates in completion of initial mental health assessments and discharge planning for Colbert class members
Regularly attends all care coordination/service planning meetings and team meetings
Completes the Comprehensive Service Plan and Risk Assessment & Mitigation Strategies with class members and monitors compliance
Meets with class members for quality compliance as outlined by DMH
Routine review of clinical record documentation
Assures all critical incidents involving Williams and Colbert class members are reported to DHS/DMH within prescribed formats and timelines
Ensures all clinical activities are compliant with standards, policies and procedures of DMH requirements for Williams and Colbert class members
Provides ongoing supervision and training to part- time quality administrator staff
Documents monitoring of the processes, services and issues for each class member
Performs all other duties assigned by the supervisor
Ability to enter, organize, and summarize data
Ability to communicate clearly and to instruct and train others
Ability to work independently on special projects
Demonstrated knowledge of adult mental illness and treatment modalities including skills groups, illness education, recovery stages of change WRAP (wellness recovery action planning), and treatment approaches with dually diagnosed individuals
Knowledge and skills in problem identification and analyses, solution development and quality assurance
Working knowledge of Rule 132
Ability to complete behavioral health assessments and treatment plans
Ability to work cooperatively a member of a multidisciplinary team
While performing the duties of this job, the employee regularly works in an office setting
Roughly one quarter of the employee’s time will be spent in the community: driving to and from client homes and nursing facilities, completing assessments at local nursing homes, and providing quality assurance services in client's homes
While in transit, the employee may experience changing road and whether conditions
In supporting clients to live independently, employee may be exposed to varying home conditions
While performing the duties of this position, the employee travels by automobile and is exposed to changing weather conditions.
While conducting external KYC business or traveling between sites, the employee may be exposed to fumes or airborne particles as well as changing weather and road conditions.
Will be required to drive daily for meetings and/or visits to residential facilities. Will work in varied residential environments based on assigned caseload.
Master’s Degree from an accredited college or university in social work, psychology or a related discipline.
Qualified Mental Health Professional or LPHA
3-5 years experience working with individuals with serious mental illness
Computer proficiency, including knowledge of Word, Excel and Internet skills
