Norwood Crossing is a nonprofit healthcare organization specializing in quality senior care. Our facility is located on the northwest side of Chicago in the quaint neighborhood of Norwood Park. Our organization is seeking a full-time Social Services Coordinator to join our team. Our ideal candidate will pride themselves on displaying professionalism, providing top notch care, and having the enthusiasm and willingness to learn.
Social Services Coordinator Job Summary:
Provides residents and families with the support needed to transition to the facility and the aging process.
Also, provides care and case management or interventions designed to promote person centered care.
Tuesday - Saturday - 9:00am to 5:00pm
(Additional hours may be required to meet the needs of the facility)
Completes and documents initial resident assessments for all admissions and readmissions and all required ongoing assessments.
Assists new resident and family with orientation to community.
Advocates for the resident and family during the resident's entire stay with Norwood Crossing including admission and discharge.
Researches and provides information on services to family members in need of additional support and resources.
Participates in Care Plan Conferences.
Coordinates problem-resolution process across departments that address resident, family or staff concerns.
Discusses current Medicare residents' plan of care and plan for discharge on a regular basis. Plans discharges for residents by working with the resident and family to determine best discharge options.
Makes appropriate referrals to community resources.
Proficiency in advanced directives.
Qualified candidates will also possess:
Bachelor's degree or related work experience
Excellent customer service and communication skills
Excellent organizational and follow up skills
Proficient with Word, Excel and Outlook Experience with electronic medical records a plus
