Oversees and facilitates care plans for sisters living independently, as well as in assisted living situations throughout the province
Assists sisters during times of acute illness, performing clinical assessments to define appropriate time course and level of care needed
Assists sisters who normally live outside Carondelet Center during times of care transition (home, hospital, rehabilitation facility, Carondelet Center). This may require travelling to visit sisters in hospitals, nursing homes and other care facilities.
Acts as liaison with Holy Family Home Care staff during transitions in and out of Carondelet Center.
Works with sisters to develop their own care plans to prevent disease exacerbation, improve outcomes, increase their engagement in self-care, decrease risks, and improve physical, mental, emotional and spiritual health.
Assists sisters in navigating the health care system, coordinating their specialty care, following-up on test results and other care coordination needs.
Acts as resource to health and wellness liaisons in Bay Area and in Hawaii, providing consultation and direction as needed
Supervises other members of health and wellness team such as home health aides and/or drivers, drafting schedules and directing them to areas of need defined in individual care plans.
Bachelor’s degree in nursing, master’s degree preferred.
Current unrestricted California registered nursing license
Able to demonstrate strong clinical nursing assessment skills
Minimum of 3 years of related experience in an environment where clinical nursing skills as well as care coordination and care management skills were required
Good communication skills, both verbal and written
Experience in at least one position working with geriatric patients
Some understanding of the culture of women’s religious communities.
