Hebrew Home at Riverdale is looking for a full time Case Manager to join our dynamic team.
With the guidance/support of LMSW, case worker will work together with the team to monitor and provide services to homeless seniors who are medically frail with chronic history of mental health diagnoses. Responsibilities include but are not limited to, community outreach, provide support to tenants, complete psychosocial assessments, work with LMSW in admission process, provide crisis interventions, provide progress notes every 60/90 days, complete ISP for all tenants and ensure goals are being met, assist tenants with appointment making, build relationships with vendors such as MLTC/Medicaid office/SS office / SNAP offices.
Ideal candidate must possess a BSW or MSW from an accredited college.
Must have experience on working with seniors who are medically frail;-some knowledge on of working with chronically mentally ill/geriatric population, homelessness, -bilingual Spanish speaking preferred. Candidate must be detail oriented, possess excellent oral/written communication skills and superior interpersonal.
