The duties of this position are in support of the mission, goals and objectives of OHSU, IDD and OCCYSHN as well as MCHB Title V public health goals and objectives. The position operates primarily at the policy level and has three broad areas of focus:
Promote and support excellence in cross-systems care coordination for CYSHCN at the state, county, community and practice levels.
Lead efforts to develop and implement statewide strategies to address the Title V Block Grant priority focused on Medical Home.
Support accountability of entities implementing and improving CaCoon (CAre COordinatiON) public health nurse home visiting in meeting program standards.
