VCU Health System's Va Coordinated Care is seeking a full time RN Liaison Coordinator to coordinate the care of patients by assisting them to achieve the most appropriate level of care, at the most appropriate place and at the most appropriate time.
The RN Liaison Coordinator will help the patient navigate through the healthcare system appropriately utilizing services and community resources. Responsible for direct supervision of assigned clinical, support and administrative staff assigned to the VCC Program
Baccalaureate Degree in Nursing from an accredited School of Nursing
Current RN licensure in Virginia
AHA BLS HCP certification or equivalent
Minimum of five (5) years of relevant clinical nursing work experience including two (2) years in a
Minimum of two (2) years of supervisory/management experience
Master’s Degree in Nursing from an accredited School of Nursing
Two (2) years of work experience performing disease or case management interventions
