Hospice of Yuma is seeking a full-time Director of Patient Care Coordination (Registered Nurse) to join our team of Hospice professionals. We pride ourselves on providing the most comprehensive and compassionate care to our patients and their families coping with end of life illness. As the only non-profit hospice in Yuma County, we have served the community for 36 years and have been voted as the Best Medical Service Provider for Yuma for the past 3 years.
As The Director of Patient Care Coordination, you will:
Coordinate and oversee all direct and indirect patient/client services provided by clinical staff.
Define and communicate expectations regarding quality provision of services and productivity to interdisciplinary team to assist them in continually improving all aspects of hospice services.
Provide regular communication with the hospice staff, including clinical staff, regarding pertinent organizational and clinical issues.
Be responsible for the oversight of the assessment and evaluation, intervention, and documentation of patient and family needs.
Ensure Interdisciplinary Team (IDT) staffing meets the care and quality standards of the organization.
Provide leadership to the interdisciplinary team promoting consistent performance improvement, teamwork, and leading the team by example.
Assure compliance with local, state and federal laws, Medicare regulations, HIPAA, and established Hospice of Yuma policies and procedures.

Requirements for this position include:
A graduate of an accredited school of nursing. BSN or higher degree required.
Must have at least 2-3 years of hospice or home health clinical supervision experience.
Current licensure in good standing as a Registered Nurse in the State of Arizona.
Thorough understanding of Hospice Medicare Conditions of Participation.
Excellent verbal and written communications skills.
