SUMMARYCoordinate delivery of quality care to geriatric residents in our assisted living community to meet daily living needs and promote wellness.
ESSENTIAL RESPONSIBILITIES: 1.Maintain quality assurance of entire assisted living community to meet expectations set forth by Bellaire. 2.Coordinate management of all healthcare needs on residents in assisted living community with physicians and other healthcare providers as needed. 3.Meet with every new resident and complete an initial health assessment prior to them moving into the facility. 4.For at-risk residents, coordinate the assistance plan, activate resources through home health, and communicate with the physician regarding issues that keep the resident at risk. 5.Respond to resident emergencies in a calm and professional manner while adhering to Bellaire policies and procedures regarding resident emergencies. 6.Administer medications to residents as ordered by physicians per the Medication Administration Record and document appropriately to comply with Virginia regulations regarding proper procedures for administering medications. 7.Supervise Medication Aides to ensure proper administration and documentation. Report any inconsistencies to Executive Director. 8.Supervise Direct Care staff to ensure all resident needs are met in a timely, courteous, professional, and caring manner. Monitor and approve documentation to ensure completion of assigned tasks by Direct Care Aides. Report any inconsistencies to Executive Director. 9.Routinely monitor resident appearance and hygiene throughout the day to maintain resident dignity. Direct staff as needed to rectify any issues relating to resident care and hygiene. 10.Ensure vital signs are acquired monthly on each resident. 11.Provide basic foot care to residents as needed to maintain nail hygiene. 12.Provide in-service training as needed for staff on topics related to daily care of residents with dementia so they may better attend to resident needs and address behavioral challenges. 13.Assist with developing an assistance plan for each resident upon moving to the community and as necessary to respond to changes in resident needs. 14.Supervise Innovations Activities Assistant for completion of activities as scheduled on monthly activity calendar. 15.Assist with scheduling as needed to ensure proper staffing levels. 16.Complete additional tasks as assigned by the supervisor.
WORKING BEHAVIORS: 1.Adhere to and carry out all Bellaire policies and procedures 2.Adhere to and carry out all regulations set forth in the Virginia Standards for Licensed Assisted Living Facilities. 3.Adhere to Code of Virginia 63.1 –1 182.1. Rights and responsibilities of residents of assisted living facilities 4.Report on-the-job injuries to the supervisor before the end of the work shift on the day the injury occurs. 5.Establish and maintain constructive working relationships with coworkers, residents, families and visitors. 6.Maintain confidentiality of verbal and written information pertaining to residents, facility operations and personnel. 7.Promote Bellaire at Stone Port in a positive way. 8.Promote teamwork in providing services to residents.
KNOWLEDGE EXPECTATIONS: 1.OSHA-MSDS sheets and procedures 2.Fire and safety procedures 3.Local referral resources 4.Nursing standards of practice for older adults
WORKING CONDITIONS: 1.Heated/Air-conditioned, well-lit facility with minimal barriers 2.Requires ability to stand or walk up to 4 hours in an 8 hour period 3.Requires ability to sit up to 4 hours in an 8 hour period 4.Requires lifting up to 10 lbs. frequently 5.Requires lifting 25-50 lbs. occasionally 6.On call on a rotation schedule
QUALIFICATIONS: 1.Nursing license in the state of Virginia 2.Current CPR certification 3.Experience in working with older adults with dementia 4.Strong health assessment skills
