We are building an amazing team of Pediatric Nurses within our Home Health Department and are looking for someone with nursing leadership experience to coordinate care. This position is open to qualified RN's, with a valid nursing license (BSN preferred).
The Home Health RN Care Coordinator is responsible for the coordination of patient care and the authority to coordinate the delivery of services provided to Home Health patients. This position functions in a collaborative partnership with the patient, physician and the entire care team in the Home Health setting. The Care Coordinator also serves in an advocacy role to ensure the delivery of quality, efficient, and cost-effective patient-centric health care services.
Education: Bachelor Degree in a health related profession, Diploma in Nursing required. BSN preferred.
Experience: Minimum of 3 years in clinical experience, HH case management experience preferred.
Training: On the job training provided, including disease specific or clinical certification education
License: Current Florida RN License
Other: Proficient computer skills, including Microsoft Office (Word, Excel and PowerPoint) required. Knowledge of adult learning principles and education methods required. Exceptional communication skills and ability to develop and maintain effective relationships with staff and care team required.
