Advanced Practice Registered Nurses / Physician Assistants
Evaluates and consults based on patient’s needs including: pain and symptom management, goal setting and clarification of treatment options, advanced care planning and discussion regarding illness and end of life issues. Collaborates closely with team members in making recommendations for patient’s plan of care.
Assesses patient health status including obtaining relevant health and medical history and current physical or psycho-social complaints.
Performs physical exams and formulates diagnosis based on this information.
Collaborates with other healthcare providers and agencies to provide and coordinate services for patient/family.
Prescribes medications, including controlled substances, to the extent delegated and licensed.
Addresses patient’s daily nursing concerns, and educates staff regarding plans of care.
Conducts face-to-face visits and completes documentation for recertification.
Prepares and maintains accurate patient records and documents to support medical practice and reimbursement of services provided.
Completes billing sheet, using appropriate CPT and ICD codes for each billable visit, and other hospice documentation as required.
Attends quarterly clinical case review and presents appropriate cases for review
Other duties and responsibilities as assigned.
Minimum Education: Graduate from accredited nursing school. Must have prescriptive authority.
Minimum Experience: Two years experience as Nurse Practitioner in hospice, home health, or long term nursing care.
Required License: Current Colorado RN license. CPR Certification. Colorado state driver’s license and proof of insurance. CPR Certification.
